Our healthcare team works together to provide accessible, inclusive, and person-centred care that reflects the needs of our diverse community. Eligibility
Health Services are available to rostered patients of Sandy Hill Community Health Centre.
Because many people in the community are seeking a primary care provider, SHCHC works with our Ontario Health Team and community partners to coordinate access to care.

Referrals to specialists or community services

Access
Sandy Hill Community Health Centre is accepting new primary care clients through Health Care Connect and other established community referral pathways who:
Live in the K1N postal code area
Have a valid Ontario health card (OHIP)
Do not already have a Family Doctor or Nurse Practitioner in Ottawa.

To get started, register with Health Care Connect at 1-800-445-1822 or visit the Ontario Health website. If you want to become a client and have trouble registering with Health Care Connect, email pcintake@sandyhillchc.on.ca or call (613) 789-1500 for help. We are not able to accept direct requests for primary care through our website, by phone, or in person at this time.
