Outreach

Engages directly with people experiencing housing instability on the street, in encampments, and at community partner locations. The Outreach Team is trained in crisis de-escalation and outdoor safety response, providing immediate support during high-risk situations and connecting individuals with wraparound care through SHCHC for ongoing follow-up. The team supports individuals in reaching their health and recovery goals through navigating recovery pathways, referrals to health and social services, and recreational activities.
