Nominations open for SHCHC Award for Excellence in Health Promotion

Recognizing outstanding contributions to the promotion of health in our community, and those who have supported others during the pandemic!

With the annual Award for Excellence in Health Promotion, the Sandy Hill Community Health Centre (SHCHC) is proud to honour those whose activities and initiatives have contributed to improving the health and wellness of our community. This year, SHCHC will present two Awards of Excellence in Health Promotion, to pay tribute to those individuals or organizations who have supported others during the COVID-19 pandemic. Eligibility 

Nominees for the SHCHC Award for Excellence in Health Promotion must be:

  • individuals who work or reside within the SHCHC catchment area (Sandy Hill and Ottawa East);
  • businesses and organizations that operate in our catchment area; and
  • the nominee, business or organization must not have any conflict of interest with SHCHC that cannot be mitigated.


Award criteria

To qualify, nominees must demonstrate leadership activities that:

  • supported our community during the COVID-19 pandemic;
  • are relevant to the SHCHC mission and current strategic priorities;
  • strengthen people’s health knowledge and the skills required to prevent ill health, and to enhance and protect healthy behaviour;
  • create and sustain environments that are supportive of health;
  • advocate for health to gain political commitment, policy support, social acceptance or system support for a particular health goal or program; and
  • have a current or potential impact on determinants of health that are relevant to the community served by SHCHC (e.g. individual knowledge and skills, healthy environments, or health policy).


Nomination forms


 Each nomination must include the following:

  • Nomination Form and Declaration of Conflict of Interest signed by the nominee.
  • A 150-200 word personal or corporate profile that describes the nominee’s specific role in the initiative, the extraordinary actions they have taken in the past year to support the community during COVID-19, as well as the motivation for their efforts; self-nomination is permitted, but must be supported by another person named within the narrative justification.
