If you hold an OHIP card and are looking for a family doctor or nurse practitioner, please register with Health Care Connect at 1-800-445-1822. Our clinic selects new patients from the Health Care Connect waiting list as spaces become available.

Health Promotion & Chronic Disease Management
We assist members of the community to address health concerns and obtain the information they need to make healthier choices and develop skills for a healthy lifestyle. Other services offered include health promotion and wellness activities, chronic disease prevention and management programs, physical activity and active-living programs, and health information workshops.
