To address a high volume of new client registrations, our primary healthcare clinic is unable to accept new patients at this time. If you hold an OHIP card and are looking for a family doctor or nurse practitioner, please register with Health Care Connect at 1-800-445-1822. Our clinic selects new patients from the Health Care Connect waiting list as spaces become available.
We sincerely apologize for any inconvenience this may cause and thank you for your understanding.
Health centre boards are empowered through the Ontario Business Corporation Act (1990) to carry out their duties and responsibilities. Community Health Centre (CHC) boards in Ontario have the same legal responsibilities of financial viability and quality of care and service as a hospital board, but do not have the formalized structures and processes by legislation (i.e. the Public Hospitals Act). Rather, CHC boards must ensure that quality improvement and financial management responsibilities are met. While CHC boards lack a legislative framework, they have the benefit of control over policy, programming and financial decisions.
Community Health Centres reflect the understanding that health is within the control of the individual and the community. CHCs are governed by a community-based board to ensure accountability and responsiveness to the needs of their own particular community. Board members represent community residents, clients, and generally reflect the population of the community served. They may also be professionals and others who have expertise in some areas, such as financial management or health policy.
Recruitment is typically held every year in early spring, to allow the Nominations and Governance Committee enough time to select the eligible candidates for election at the Annual General Meeting at the end of June.
An online application form is available DURING recruitment periods. Please note that the call for nominations for the SHCHC Board of Directors is currently completed for the year, and we are not accepting further applications. The Board is at capacity, and the recruitment process is closed for the year. We will keep the community informed when nominations are opened in the future.

Additional eligibility requirements may apply for specific committees and information is posted during recruitment periods.
The Sandy Hill Community Health Centre is committed to advancing equity, diversity and inclusion in every aspect of its work, including the appointment process on the Board of Directors. As such, SHCHC urges Indigenous and racialized persons, persons with disabilities, and women and gender diverse persons to apply when recruitment is open, and to include, on a voluntary basis, how their lived experience and perspectives strengthen their application in this regard.
The Nominations and Governance Committee of the Board (NGC) is responsible for supporting the recruitment of potential Board members. Applications are collected and provided to the NGC. The committee is responsible for reviewing applications and making recommendations to the Board on who to appoint, based on the Board Nomination and Election Policy.
All applications are held on file for the term of the Board. If applicants are not appointed initially, they may be contacted later in the term if a vacancy arises, if they wish to be considered at that time.
